Adult Evaluation & Review Services
During these hard economic times, many people are seeking assistance that will not break their budget. The Adult Evaluation & Review Services (AERS) is a free evaluation program for Garrett County residents age 18 and older which matches clients with services for which they are eligible. AERS core staff, including a registered nurse and a licensed social worker, meets with the client and their family in their home or other location to develop a plan of care.
AERS staff evaluates service eligibility such as:
• Nutrition Referrals: to home delivered meals or consults regarding nutrition
• Medication Assistance: Counseling and referral regarding Medbank, and other medication programs
• Transportation referrals for special transportation funds (ACS) or counseling on GTS services
• Referrals for medical equipment needs
• Referrals and counseling regarding Medical Day Care
• In-home Aide Service (non medical): In Home Care Provider list maintained by AERS and information on private options
• Nursing Home Transitioning; assisting in the process and referrals to the nursing home of your choice
• Counseling and referrals regarding housing options.
• Referrals for Respite options
• Social Security Benefits/Medicaid Application Assistance counseling and assistance with the process
• Other referrals as requested
