Special Health Care Needs
Nurse Case Management is a free care coordination service. The service will be provided by an RN.
What will a Case Manager do?
The nurse will be available to assist parents, caregivers and providers in accessing health and other needed services. This may include:
Assisting families obtain needed medical equipment
Helping parents understand and/or implement special care instructions for the child in a home setting or the school setting.
Referring familes to other needed resources.
Who is eligible?
Services are available for parents of children, from birth through 18 years of age, with special medical conditions and needs. These needs can range from asthma, diabetes, ADHD to significant disability.
