This program is designed to help chronically ill and disabled persons who qualify for Medical Assistance to remain in their homes by providing assistance with some of the simple activities of daily living. Help with feeding, dressing, bathing, toileting, moving about, and care of hair, skin and teeth.
This program will pay for a Personal Care Provider and a Registered Nurse Case Monitor. The Personal Care Provider will come into the home to help with these simple daily activities. The Registered Nurse Case Monitor will plan the care given. This plan of care will be reviewed and approved by your doctor. The Registered Nurse Case Monitor will make home visits to check on the patient and to evaluate the plan of care.
